General Information
MAYSA Release of Liability and Consent for Medical Treatment form with Concussion Information
Documents for Check in:
- * CONTINUING THIS YEAR * Every team (including Madison area teams) must present a completed waiver signed by a parent or guardian for each player participating in this event. New this year: This form is two sided with important information about the risk of Concussions. Please read and sign both sides. These documents will be checked by tournament staff at team check in and must be kept on the sideline of every game during this event.
- Out of state teams MUST use this waiver. Out of state registration forms, waiver, or medical release statements will not be honored.
- Wisconsin teams may present a copy of their player registration form provided that it includes language releasing WYSA and/or MAYSA in the release of liability and medical waiver text.
Bringing a Guest Player?
Guest players are appropriate-age players registered to teams in other clubs. Up to 3 guest players are permitted per team for this event. Guest players must be listed on an approved roster from your state association:

Club Passing Players
Appropriate-age recreational or classic players from different teams registered in the same club,
are considered part of the team's formal roster and are not counted against the 3 guest player limit. A team may use up to 4 club passing players.
They may be hand-written onto the approved roster. (This does not apply to state level players,
who must always be used as 'guest' players.) Club passing players must have player pass cards at the tournament. Contact the MAYSA office if
player pass cards are needed for recreational players. Don't forget waivers for all club passing players!
Parking at Event:
There will be a fee to park in all Reddan Soccer Park parking
lots. Parking passes cost $10/weekend ($6/day) at the event. Parking passes may be purchased in advance for $8/weekend. See Tournament Forms & Documents, above, for Advanced Parking form. Please mail form and fee by the deadline to the address on the form.
