WYSA Release of Liability and Consent for Medical Treatment formDocuments for Check in:
- * CONTINUING THIS YEAR * Every team (including Madison area teams) must present a completed waiver signed by a parent or guardian for each player participating in this event.
- Out of state teams MUST use a Wisconsin waiver. Out of state registration forms, waiver, or medical release statements will not be honored.
- (MAYSA teams may present a copy of their player registration form provided that it includes the waiver/medical release language.)
Club Passing Players
Appropriate-age recreational or classic players from different teams registered in the same club, are considered part of the team's formal roster and are not counted against the 3 guest player limit. A team may use up to 4 club passing players. They may be hand-written onto the approved roster. (This does not apply to state level players, who must always be used as 'guest' players.) Club passing players must have player pass cards at the tournament. Contact the MAYSA office if player pass cards are needed for recreational players. Don't forget waivers for all club passing players!Parking at Event:
There will be a fee to park in all Reddan Soccer Park parking lots. At the event, parking passes cost $10/weekend ($6/day). Parking passes may be purchased in advance for LESS (see 'PARKING' link to left.)
